These are deliberately short. The point is not to make testers do UX homework. The point is to catch where the experience and the person disagree.

Your opinion has a job here.

If a finding causes a change, it can be logged against the cycle. Testers can stay anonymous or add a name/handle for credit when they share their results.

Find the friction.

You do not need design vocabulary. If something surprises, confuses, or slows you down, that is useful evidence.

Useful findings can become visible changes instead of disappearing into a feedback black hole.
